At Huisartsenpraktijk Rembrandtsingel, you can take care of many things yourself via the digital patient portal Uw Zorg online. Think of requesting medications, viewing test results, or asking your GP a question.

 

You can use this via a free app on your phone or through the website of Uw Zorg online.

 

Your medical data remains private and securely protected. Only you and your GP can view the information.

How do you get started?

You can use Uw Zorg online in two ways: via the website or via the app.

Prefer to use the website? Then go to your patient portal.

Click on 'Log in patient environment' and follow the steps. You’ll need your DigiD to log in securely. After logging in, select Praktijk Rembrandtsingel as your GP.

Download the app

  1. Go to the App Store (iPhone) or Google Play Store (Android) on your phone or tablet
  2. Search for: Uw Zorg online
  3. Download and open the app

Find your GP

  1. Open the app
  2. Search by practice name: Praktijk Rembrandtsingel
  3. Select our practice

Connect with your DigiD

  1. Click on ‘Link account’
  2. Log in with your DigiD (your personal digital key for healthcare and government)
  3. After linking, you will see your personal information

Requesting Repeat Medication

Do you use medication that you need for a longer period? You can request it through the portal without having to call us.
How does it work?

  1. Log in via the website or the app
  2. Click on ‘Repeat medication’
  3. Select the medication you need again
  4. Send the request with one click

Your prescription will be processed by us and forwarded to your pharmacy.

Note: Do you request your medication before 10:00 AM? Then we usually process it the same day.

View test results

Have you had blood drawn or another test done? Then you can check the results yourself as soon as they are available.

How does it work?

  1. Log in
  2. Go to ‘Results’
  3. You’ll see the name and date of the test
  4. Often there’s also an explanation from your GP

You will receive a notification as soon as a new result is available.

Ask a medical question (e-consult)

Do you have a non-urgent question for the GP or practice nurse?
For example, about a previous consultation, a symptom you recognize, or a side effect of medication?
Then you can send an e-consult.

How does it work?

  1. Log in
  2. Click on ‘E-consult’
  3. Type your question and send it. You will receive a reply within a few working days.

Note: The e-consult is not suitable for emergencies or situations that require physical examination. In that case, always call the practice.
