Registering as a new patient
Praktijk Rembrandtsingel is currently accepting new patients living within postal code areas 2902 and 2903. Do you live outside our postal code area, or do you already have a general practitioner in Capelle aan den IJssel but would still like to register with our practice? If so, we kindly ask you to briefly explain your situation in the comments field or contact us by phone.Register

Bring to your first appointment
Always bring a copy of your health insurance card and a valid ID (passport, ID card or driver’s license).
Please note: processing of new registrations
Would you like to register with our practice? You are welcome to complete the online registration form.
From 20 July to 28 August, we will temporarily not be processing new registrations. Due to the summer holiday, the renovation of our practice, and working from a temporary location, we are unfortunately unable to process new patient registrations during this period.
All registrations received during this period will be processed in the order they are received, starting in early September.
