Praktijk Rembrandtsingel will soon start using Mitz. With Mitz, you can decide whether we may make your medical information available to other healthcare providers involved in your care.
This gives you control over how your medical information is shared.
Why is your consent important?
Sometimes another healthcare provider needs information from us to provide you with good and safe care. This could be an out-of-hours GP service, a pharmacy or a specialist at the hospital.
Having the right information means that a healthcare provider can, for example, see which medicines you use or other medical information that is important for your treatment.
We cannot simply make your medical information electronically available to other healthcare providers. In most cases, we need your consent first.
What is Mitz?
Mitz is a national service where you can record your choices about sharing your medical information. At MijnMitz.nl, you can choose which healthcare providers may make your medical information available to other healthcare providers involved in your care.
Your medical information itself is not stored in Mitz. Mitz only stores your choices about consent. Healthcare providers have agreements about which information they are allowed to share.
What do you need to do?
Go to MijnMitz.nl and log in with DigiD. There you can view and record your consent choices.
You decide which healthcare providers you give permission to and which you do not. You can change your choices at any time.
Have you already given us permission?
Have you previously given Praktijk Rembrandtsingel permission to share your medical information? This permission will be added to Mitz.
This means that after we switch to Mitz, you may already see a consent choice when you log in to MijnMitz. You can view or change this choice there.
How does consent work for children and young people?
Consent choices for children and young people can also be recorded in MijnMitz. Different rules apply depending on the child's age. Your child needs their own DigiD to use MijnMitz.
Children under 12
As a parent or legal guardian, you record the choices in MijnMitz using your child's DigiD. You then confirm these choices in your own MijnMitz account using your own DigiD.
Children aged 12 to 15
Your child can log in with DigiD and record their own choices. You can also do this together. As a parent or legal guardian, you then need to approve the choices in your own MijnMitz account.
Young people aged 16 and over
From the age of 16, your child can log in with DigiD and record their own consent choices. These choices are then immediately final.
